The issue you are facing:

A file copied into a synced folder is initially synced (green arrow). After renaming it the arrow changes to the “sync pending” and the Client log keeps saying “ changed” but with the time “now”.
The rename is never synced to the server (or at least not applied) - pausing sync/restarting client nothign changed that.

I’m facing the same problem on one of my computers.
Renamin a file is not seen by Nextcloud desktop client. If I move the file out of the folder and move it back aganin, then it is seen and synced.

The bug is present in Windows client 3.12.4 and 3.13.0.
You can workaround by uninstalling the client end reinstalling the 3.12.3.
