Steps to replicate it:

Tried Removing and reinstalling NextCloud Office as well as re-pulling docker images. Unable to select radio buttons or change url of Collabora server. It’s greyed out and stuck on “https://mydomain.com/custom_apps/richdocumentscode/proxy.php?req=”. An address I’ve never entered and am unable to change. Persists after removal and reinstall of NextCloud Office app.

the URL looks the built-in CODE is installed. I would recommend you remove this app and if still doesn’t help re-install richdocuments as well. You can try occ way too
