I have shared my private calendar with some family members.
However, I don’t want to share all appointments with the family.
How can I create an appointment that is not shared? Can I mark it as “private”?

Yes, you can, from the Nextcloud Calendar web app.
Using Calendar 1.6.4 on NC 14, I get this drop-down in the Details tab for any given event on my ‘default’ calendar:
It’s not immediately obvious which version of the Calendar App added this feature, but if yours doesn’t have it, it might be time for an update.
