This. 5400 errors all about this file.
I have successfully run occ db:add-missing-indices and occ maintenance:repair --include-expensive and neither fixed it.
file_get_contents(/path/to/nextcloud/data/appdata_xxx/avatar/server/avatar.128.png): Failed to open stream: No such file or directory at /path/to/nextcloud/lib/private/Files/Storage/Local.php#327
So you have a user named “server”
Rescan the data/appdata_<instanceid>/avatar folder:
occ files:scan-app-data avatar

In my case, specifically:
sudo -u www-data php --define apc.enable_cli=1 /var/www/html/nextcloud/occ files:scan-app-data avatar
