My nextcloud instance stopped working without any change.
Nextcloud is running in a docker container all up to date. My logs now get full with this error:
nginx: [emerg] duplicate upstream “php-handler” in /config/nginx/site-confs/default.save:1
I didn’t changed anything… And don’t know what the error means

Hmm I’m no docker expert, but is there any other config file inside /config/nginx/
that might be included and had the PHP handler defined as well? E.g. some module config?

I had the same issue running Nextcloud with Dockker and a reverse-proxy in front of the nginx. Renaming the file to .default.save worked for me, it seems like the file is not needed.
